Creating Meaningful Moments, One Kit at a Time

Hospice of the Valley volunteers are assembling 840 “Meaningful Moments Kits” to be distributed through local libraries to individuals living with dementia and their care partners.

Each kit contains thoughtfully selected activities curated by Hospice of the Valley’s Dementia Care and Education Team to encourage connection, spark conversation and engage the senses. From picture books and conversation cards to puzzles and creative activities, these kits offer simple ways for families to enjoy time together.

Hospice of the Valley is grateful for our grant-funded partnership with the Maricopa County Department of Public Health’s BOLD program, which is making these kits possible and bringing resources directly into our communities.
