Compassionate dementia care.

Located at our Dementia Care and Education Campus in central Phoenix, Levine Home opened in 2022 to serve hospice patients in need of exceptional round-the-clock care. Stays are short-term until patients are stable enough to return to the comfort of home, wherever they reside. Named in honor of former Hospice of the Valley Executive Director Susan Levine, this inpatient care home serves all, but has special expertise in supporting people living with any stage of dementia, ensuring they receive comfort, dignity and compassion. Family members and pets are welcome anytime, day or night.

Prior to March 2023, patients living with dementia were cared for at Gardiner Home in central Phoenix. When it opened in 1995, it was the only free-standing palliative and hospice home nationwide. Gardiner Home then transitioned to providing specialized care for people who were living with Alzheimer’s disease and other dementias.
